Purpose
CoordiHome exists to support outreach teams, case managers, and social intervention workers serving people affected by substance use and social instability. The platform is designed to document visits clearly, preserve the context of past encounters, and help teams respond with greater continuity, safety, and care.
Founder's Narrative
While working alongside vulnerable communities, I kept seeing the same pattern: people living with substance use challenges were often engaged by multiple helpers, yet their stories had to be retold from the beginning every time. Important details from previous visits were lost, follow-up plans were fragmented, and opportunities to intervene early were missed. The issue was not only access to services. It was continuity. Outreach workers, social intervention teams, and support programs needed a simple way to understand what had already happened, what risks had been observed, and what next steps had already been offered. Without that continuity, every visit risked becoming isolated instead of part of a longer path toward stabilization and recovery. CoordiHome was created in response to that need. It is built to help teams: • Log visit details in one place • Preserve information from past encounters • Track intervention plans and follow-up needs • Surface employment opportunities as part of longer-term recovery and reintegration This platform is rooted in a simple belief: better information leads to better care. When teams can carry forward the context of each visit, they are better equipped to respond with dignity, consistency, and practical support.
Prototype Note
This current version is a prototype and stores visit information in the browser for demonstration purposes. A production-ready version would require secure authentication, protected storage, and privacy safeguards appropriate for sensitive social and health-related information.
